Pheochromocytoma and paraganglioma are catecholamine-producing tumors that arise from the adrenal glands or paraganglia. These rarely occur in chest, especially anterior mediastinum. Here, we describe a rare case of mediastinum, report on key points surgery perioperative management. An 80-year-old woman was admitted to our hospital for fatigue hypertension. A 123I-meta-iodobenzylguanidine scintigraphic scan revealed an mediastinal tumor with high signal intensity. Chest computed tomography showed 44-mm mass mediastinum adjacent ascending aorta superior vena cava.
